General Summary

We are holding a block of rooms and will be making a reservation with the Loews Chicago O'Hare Hotel in Chicago, IL upon receipt of this registration form. The cost of the meeting is $995 which includes attendee meals and meeting costs.  The cost for spouses' dinner and reception is $100 each.  The room rate has been established at $179 per night. 

The session will begin with a reception at 7:00 pm on Sunday, September 17th and will end at noon on Wednesday, September 20th.  Please plan to make your departure flight for 2pm or later on the 20th.  If you plan to come in early or stay over an extra night, please let me know ASAP as we have limited rooms reserved in our block.

All meeting fees will be charged if there is a cancellation less than a week before the session.
